Building Skills. Creating Opportunities. Promoting Independence.
Love U2 Care, LLC provides Adult Habilitation / Community-Based Services designed to help adults with intellectual and developmental disabilities build practical skills, increase independence, and participate more fully in their communities.
Support is individualized around each person’s strengths, abilities, preferences, goals, and service plan. Our focus is on helping individuals develop skills they can use in everyday life while encouraging choice, confidence, self-direction, and meaningful community involvement.
What Adult Habilitation / Community-Based Services May Include:
Working toward individualized goals identified by the person and their support team.
Supporting involvement in community activities, events, recreation, and meaningful experiences.
Building confidence with communication, relationships, social interaction, and appropriate community engagement.
Developing practical skills that support greater independence in everyday routines.
Supporting awareness, problem-solving, choice, self-advocacy, and personal safety.
Building skills related to routines, organization, meal planning, household responsibilities, money awareness, and other areas identified in the service plan.
Support Built Around the Individual
Adult Habilitation is not a one-size-fits-all service. Love U2 Care works with each individual and their support team to identify abilities, interests, goals, and areas where additional skill development may be beneficial.
Support may take place in community settings and other approved environments based on the individual’s service plan. As skills develop and goals change, support can be adjusted to encourage continued growth and greater independence.
Who This Service May Be For
Adult Habilitation / Community-Based Services may be appropriate for adults with intellectual and developmental disabilities who benefit from individualized assistance with skill development, community participation, social development, and increased independence.
Eligibility, service frequency, authorization, funding, and the specific supports provided may depend on the individual’s assessed needs, service plan, funding source, and applicable program requirements.
Helping Individuals Thrive in Their Communities
Our goal is to help individuals build confidence, strengthen abilities, pursue meaningful goals, and participate in their communities in ways that reflect their interests and choices.
